For Referring Providers

At Integrity Psychological Services, we value collaboration with therapists and other health care providers to ensure seamless care for clients. To refer a patient to us for medication management services, please complete the following steps:

1. Download and complete the referral form.

2. Include relevant information. Please provide details regarding client’s mental health history, current treatment and any previous medication trials.

3. Submit the referral: Email the completed form to medreferral@integritypsych.com or fax to 412-202-1705.​

Our office will contact you to confirm receipt and to discuss the next steps.
